Clinical Supervision Spotlights II: Feedback-Informed Treatment and Deliberate Practice for Clinical Supervisors

"Using Client Input to Guide Clinical Practice and Enhance Therapeutic Outcomes"

DESCRIPTION: 

Current approaches to psychotherapy tend to emphasize the importance of modality or technique in achieving desired client outcomes. Mental health practitioners are typically concerned with "what" to do in order to support their clients in achieving their therapeutic goals. This view is supported by clinical supervisors who often over-focus on strengthening clinician skills through listening to narrative self-reporting by the practitioner, followed by offering counsel or guidance as the key to improving client outcomes.

The research on client outcomes tells a different story, though. Indeed, reaching consistent and robust client outcomes is more a function of "how" the mental health practitioner interacts with the client rather than what types of skills or interventions are used. For instance, the research on common factors that lead to significant change across all behavioral health modalities indicates that relational factors between the practitioner and client have an average impact of about 30%, whereas clinician modality or technique only accounts for about 15% of the change - the same amount of change accounted for by hope, expectancy, and other placebo-like variables. The greatest amount of change, at about 40%, is attributed to factors occurring in the client's life outside of the therapeutic relationship.

This is where Feedback-Informed Treatment (FIT) and Deliberate Practice (DP) can be helpful. FIT is an evidence-based approach to evaluating and improving the quality and effectiveness of the therapeutic relationship developed by psychologist Scott D. Miller, PhD. FIT tracks both the overall results of the therapeutic work across sessions along with the quality of the therapeutic relationship within each session so that modifications can be made early and more easily. Deliberate Practice is an approach to skill mastery that relies on practicing at the edge of one's ability using regular and active feedback spaced across time to achieve mastery. Used together, they help practitioners, managers, and supervisors better tailor services to fit the needs of clients and thus improve their therapeutic outcomes. Registration for this course is complimentary and open to all mental health practitioners.

Case Conceptualization and Documentation: An Essential Skill for Addiction Counselors

"Organizing Clinical Information in a Meaningful Way to Track Client Progress and Support Service Planning"

DESCRIPTION: Along with Principles of Addiction Treatment and Addiction Counseling Skills, Case Conceptualization and Documentation, is considered to be part of the “Core Curriculum” and basic to the foundational training of an addiction counselor. This class is designed to provide the counselor with an understanding of the clinical record and the continuum of client care that the record documents and tracks. The class presents screening, basic assessment and evaluation, Substance Use Disorder (SUD) diagnostic criteria, ASAM patient placement criteria, introduction to treatment planning, progress note completion, documentation requirements, and basic referral and discharge planning. It emphasizes the confidentiality of the client record and includes releases of information, mandatory disclosure, and informed consent among other things. DESCRIPTION: This class is designed to provide an overview of the specialized models of counseling for behavioral health environments that provide services for clients with co-occurring mental health and addiction concerns. DESCRIPTION: The central nervous system (CNS), made up of the brain and spinal cord, plays a key role in how our clients perceive, experience, and respond to the world around them. Though hereditary elements and biological predispositions have a significant impact on the functioning of the central nervous system and the subsequent experience of the self, it is also incredibly influenced by a variety of environmental factors such as early experience, social learning, schedules of punishment and reinforcement, current stressors, and availability of resources such as nutrition and medical care. Because all psychotherapy is essential working with the experience of the self - vis-a-vis the reshaping of the central nervous system - it is incumbent on all of us as mental health professionals to have a foundational understanding of the key structures and functions that comprise the central nervous system, how they work together and with the other systems of the body to regulate the individual, and how they can be influenced through psychotherapy to help our clients more effectively reach their mental health-related goals. Through engaging and experiential activities, learners in this training begin with exploring the philosophical relationship between the brain and the mind. They then gain a better understanding of the structures, systems, and functions of the brain and how these relate to the four core theoretical forces in psychotherapy: 1) analytic, 2) cognitive/behavioral, 3) humanistic/existential, and 4) contextual/constructivist. In addition, this training also focuses on applied neuroscience as it relates to the treatment of anxiety, depression, stress and trauma, and addictions and substance use disorders. "Digging for Treasure - Together"

DESCRIPTION: This class of skill development in Advanced Motivational Interviewing (MI) will initially offer a brief review of the fundamental spirit, processes, and skills of MI, and then focus intensively on clinical skills development. The training is designed to assist the counselor in developing and practicing skills to assist clients in identifying and strengthening intrinsic motivation for positive change. This is an advanced course in Motivational Interviewing, so it is expected that the participant will already be able to demonstrate the foundational MI skills of conducting a client interview using the OARS + Informing and Advising microskills as well as responding effectively to no‐change talk (Resistance, Sustain, or Discord). The format will be interactive and experiential in nature. Participants will be afforded an opportunity to obtain focused practice and will receive individualized feedback regarding their MI skills with student and trainer ratings.
